MANY BANKS HAVE THEIR OWN WAY OF WRITING 'POD' ACCOUTS. 

FOR EXAPLE  FAILED BANK WAMU DID NOT WANT TO WRITE POD AFTER YOUR NAME THOUGH NAME IS IN COLUMN "TITLE" 

WHY AFTER 70 YEARS FDIC DID NOT COME UP WITH UNIFORM FORM.
