In reviewing the options provided, we prefer Appendix A. Our company does not share any information with unrelated companies, so our form is already short and concise.

The form Appendix A, or something similar, is very easy to understand. Any movement in this direction will benefit our whole industry, as hard-to-read legalese not only confuses many consumers, it gives the appearance that we are trying to hide something, which builds distrust among consumers towards us.
